This site is a patient information directory. It does not contain a medical intake form, booking checkout, account registration or newsletter collection.

Directory filters run in your browser. Do not enter medical records, identification numbers or other sensitive personal details into the search field.

External websites

Clinic links open websites operated by their respective providers. Their privacy policies govern information you submit to them. This site does not receive appointment details submitted on those websites.

Analytics

We use Google Analytics to understand visits and improve this website. Google Analytics uses cookies and collects usage information such as pages viewed, device and browser information, and interactions. Learn more in Google’s privacy policy.

Technical requests

The hosting platform handles normal page requests. Fonts may be loaded from Google Fonts, and any externally hosted provider images are requested from their original image hosts. Those services receive the technical information needed to return the requested resource.
